What to Bring to Your First Appointment
All new patients need to fill out a new patient form to share information about their oral health history, medications they may be taking, and any recent surgeries or other health issues. We encourage patients to bring the following to their first appointment:
Insurance information
Copies of dental and medical records
Recent x-rays
What Should I Expect?
